CROS 3.0 Questionnaires

Here are some examples of questions from the various domains of CROS. Questions parallel each other so that scale scores or responses to individual items derived from the consumer report, staff report or VIP report may be directly compared and contrasted.

Treatment Satisfaction (A Consumer Only Scale)

• How do I feel about the choices I get about my care?
• How do I feel about the amount of activities or groups I am offered to help me cope with my mental illness?

Hope for the Future

• I believe I will feel alert and alive.
• I believe I will achieve goals I set for myself.

Daily Functioning

• I plan and keep a daily routine on my own.

• I make plans to do things with other people.

Coping

• How well am I coping with feeling tense or anxious?

• How well am I coping with external stressors (for example, health problems, family conflicts, unemployment, tragedy in community)?

Quality of Life

• How do I feel about the amount of freedom I have where I live?
• How do I feel about my relationships with family/significant others/friends?
